Filthy, corners of bathroom had dirt buildup, trash cans overflowing. No clean bedsheets, blankets, towels available. Covid protocols not followed. Left in room with Covid patient 24 hrs. Quality of food, lacking in nutritional value. Cassidy only nice CNA there. Left with excellent wound care protocol but also left with Covid. Your reply is ridiculous. Now you want to talk to me. You didn’t while I was in there. Where are my discharge papers?

IF I COULD GIVE THIS PLACE A NEGATIVE STAR I WOULD. THIS PLACE IS FULL OF INCOMPATENT PERSONNEL WHO FROM ADMISSIONS THROUGH THE DR.S, PA.S, CASE MANAGERS, AND NURSES WHO TRY SO HARD HOWEVER THEY DO WHAT THEY ARE TOLD, HOWEVER, WHERE IS THE LOVE FOR THESE PEOPLE WHO ARE TRYING TO HEAL. I HAVE SEEN THEM LOOSE PATIENTS, NOT TAKE PATIENTS TO THEIR DRS APPTS ,BUT THEY HAVE A HUGE ACTIVITY BOARD THAT THEY ARE VERY VIGILIT ON GETTING WHOMEVER OUT ON THOSE FIELDTRIPS. I HAVE SEEN THEM NOT KNOW THAT A PATIENT WAS DISCHARGED EVEN. 4 DIFFERENT NURSES CAME IN LOOKING FOR HIM. I AM REPORTING THIS PLACE FOR MULTIPLE HEALTH VIOLATIONS AS WELL SO I WOULD NOT LEAVE MY LOVED ONE THERE IF I COULD HELP IT IN ANY WAY. MONCINETO IS EVEN WORSE.

After a horrific experience at a facility last year I was very apprehensive of any skilled nursing facility but I had a ver good experience here at citadel! The nursing staff was amazing a shout out to cass, Kai, jess, Jackie, Rachel Renee,andrea(. She would gift me merch and coke zero’s lol). Madea was such a blessing!The therapy team was top notch and really pushed me while making it fun! Ty Beth Emily, Stephanie, Lisa, Chrissy! I am really sad to have to leave. Ty to all the staff for making me feel like fam oh I forgot yurii in the kitchen.

The staff and nurses at citadel post Acute was great. Tauna Royce and Julio Meza was awesome nurses they took great care of my Son while he was there.
