The Plano office is inside the Legacy Medical Village office building at 5425 W Spring Creek Parkway. Suite #145 near the South Entrance. Our North Dallas office is located in Pavilion #1 of the Medical City Heart Hospital at 11970 N Central Expwy. Suite #400.
Booking was pretty quick and there are forms online which can downloaded and completed prior to an appointment.
