E-visa is online only and it takes a few days, I think up to five days.

The 2 year eVISA application will take 3 days to process, and the 10 year VISA can take up to 2 weeks. As far as applying for the VISA, yes, that should be enough to at least apply. Call the consulate in LA as they have people who will answer questions over the phone.

Yes, unless you do the online one, which is much easier, takes less time and is only $40ish. It's only good for 90 days for each year and called for two years. So if you're not in Brazil for longer than that, go with the e-visa.
It depends on which documents, there are emails you can ask, over the phone they don't give you any information.
They do not take appointments. You will walk into the front doors, go to the info desk, present an ID for a pass and go to the elevator they direct you to. On the eighth floor, follow signs for the Brazilian Consulate. Take a number tag from the passport window and wait for your number to be called. They are very helpful, but you MUST have all required documents. The wait isn't long usually, but it is best to get there early in case you need to get other documents.

Yes, i don't remember the exact cost but it's around 100$ and takes about 2-3 weeks to get. You have to go to the consulate with your passport. Their website should tell you the rest
Get yellow fever. You don't need it to get in, unless you are not a us citizen, but you probably don't want to catch it down there. By the way Rio is beautiful just don't be afraid when you see the police standing out with rifles, they are really nice and took a picture with us. Also brush up on Portuguese
