Same situation with physical therapy and rehab. Once again, promised a lot of things, turned out to be a complete lie and left us hanging completely at the time of discharge with little to no explanation on home care. The social workers wiped their hands clean and said they are no longer able to help out. Don’t take anyone here.

We were promised a lot by staff. Not1 promise was delivered. Very poor communication on physical therapy status . We were told we would be called every day how he was doing - it never happened . Notes on his progress was poorly updated in system. Case worker say they take care of everything and hard to contact after 1 day. Very disappointed on how the treated my 84 yr dad who just a major stroke . Find another place

Please mail the package to St. Charles Hospital, 200 Belle Terre Road, Port Jefferson, NY 11777 Attn: Receiving. Please be sure to include the patient's full name and room number/floor number. Please keep in mind that St. Charles cannot be responsible for packages that arrive after patients have already been discharged. Please be sure to coordinate closely with the patient to ensure he/she is still admitted to St. Charles during the expected delivery time.
