This place is horrible my mother was sent there for rehab she got Covid really bad she stopped eating and she could not walk they were supposed to get her up and walking they never did she sat in her room or in the common area she never ate any of the food she cannot eat on her own somebody has to feed her they never bothered to do that she cannot drink on her own anytime we went there she was extremely thirsty because nobody could hold the cup for her so she could drink she fell out of the wheelchair I don’t know how many times and they did nothing her legs are all black and blue and bruised they never bothered to call the doctor to have her checked out then they released her to my father who is handicapped and I cannot take care of her this is not a rehab facility this is a place where they just stick the people in their rooms they don’t check on them and they do nothing with them do not send any of your family members here this place is absolutely horrible it needs to be shut down

My father was brought to this facility on a Thursday evening and was supposed to have rehab starting on Monday. The Head nurse assured us that he was being taken care of and how great the place was. My sister mother and myself were there every day. Nobody gave him a bath… He could not eat his food on his own and they just left it there on the tray didn’t even check on him. When we were there on Sunday my father did not look well each day he was declining and I would call the nurse and she would assure me that they’re taking the best of care of him and that I was just overreacting. Due to the Covid rules we had to leave by 7 PM I believe somewhere around there. I wanted to take my dad out that day but he said he was willing to stay one more night to get rehab on Monday. They left him sit in a wheelchair the whole time he was there which was horrible for him. Approximately two hours after we left my father called me from his cell phone thank God he had one on him and said that his breathing wasn’t right. I told him don’t panic I’m going to call 911 and get you out of there. I first called the front desk and told the girl what’s going on with my father and she said the doctor will look at him tomorrow. I told her that’s not going to work you need to call 911. She said she can’t just call 911 without the doctor saying it’s OK. I told her if she doesn’t call 911 I’m going to call the police. She then said hold on came back to the phone and said 911 is on their way. I called my father on his phone and told him that the 911 was on the way. The next call I got to my phone was from the paramedics telling me that my father‘s pulse ox was at 75 and he was in a lot of trouble they were going to take him straight to the hospital. They immediately gave him a breathing treatment which the facility would not do. My father came very close to dying. He had pneumonia and they let him sit there all weekend reassuring me that we were over reacting. If he was not conscience enough to call me he would’ve died. This place is reckless, dangerous, not somewhere I would ever take anybody you care about. It might look pretty when you walk in but it is a place of neglect and dishonesty. My father wound up in ICU for several days. The hospital emergency room told me that they get many patients from this facility. I cannot express enough do not ever use this place. I did report them to the ombudsman. As time has passed I am considering taking legal action. I would never want this to happen to anybody else and feel very blessed that I still have my father.

Our mother was brought to The Avenue for rehabilitation after a fall. At the end of her allowed medicare days they said we would have to pay cash until we could find her another place to live.. But then she developed an infection and had to have IV antibiotics. Then she fell again. We were told Medicare would now allow additional days. We were then billed $1200 for those days. She fell 4 times while she was there each time needing a CT scan and ambulance ride to the hospital. The hospital found clots in both her legs and her lungs. She also had bed sores from being left in the bed and wheelchair for too long. I tried to address these issues with a nurse named Sandy (??) and she got defensive and rude. I placed a call to the director to call me back but never got a reply. I also told the front desk person I needed a call back and told her what happened also. Still no response. This is a state-of-the-art facility but don't be fooled."

With due respect to all the heroes. Night shift LPN tatiyana something was so rude, very confident on slandering. Another gentle lady introduced me saying she is from administrative and address very nicely before admission. But no one showed up to help my mother all night when she press the button asking help. And I checked at front desk that I wanna speak to her describing her physically but she said the administrator didn’t look like how I described. Totally a lie.Even they will not let you get in early morning or later evening because they don’t want to show the family members the terrible situation of the loved one who is staying there. They will let you in after cleaning all the situation of the clients room. And sugarcoat the family members when they arrive. So sad 95%of the clients I see wasn’t able say their situation because of their health condition.One night I stayed there, I hear lot of patients screaming asking help no one was there to help any. I feel so pain by that situation. If they are shortage of staff why they are taking clients out of capability. I was lucky to take my out from this hell. I hope the management could definitely change their facility hiring more good and caring peoples giving better care whoever lives there not showing their front entrance like a mall.
