No garlic bread with my pasta, but I was still charged with it. The waiter's mind had checked out long before he did. Worst experience I've had there ever. Been going there since I was a kid.

This is a very busy restaurant and it is short staffed like many other places. Sometimes they are literally covered up and cannot get to the phone.
Not exactly sure. Brought my kids here since they were little. They are grown now. So it been open many years. Best pizza around.
