This place is the worst. Let left my patient at the hospital never called. Never went inside to see if the patient was okay. They left her four hours away from her home. I'm very disappointment in how they treated my patient.

No show 4 of the last 5times . People are very nice just a no show problem. Perhaps a marketing of small agencies would provide the service needed in smaller towns. Up date I was told " We just don't have drivers in your area." So we are left with no service to Emory. Must go to Emory for eye care.So we have to pay out of pocket or loose what little sight that's left.

The driver didn't even call to let me know that they couldn't pick me up . I called to check a ND found out ty h r vehicle broke down. I understand that things happen, but they could have at least called.

Been waiting 2 hours keep calling they keep saying call back in 10 mins if noone shows up they cant stay on the phone then told me thanks for being a loyal rider. They do it on purpose they do not want people to actually use the service. They told me i cancelled the trip and i never did. I told them i wanted to cancel going to pharmacy and they cancelled from taking us home. Been waiting with my 2 yr old dont use their service unless you really need to. They dont care about your safety. This has happened before took my daughter to the dentist we never got picked up were there so long the dental office closed said we had to go outside it was in a area that the lyft driver told me was “sketchy”. A dentist offered us a ride home but her boss said if something happened she would be in trouble so she didnt and we waited outside luckily the transportation agency showed up finally.

It's not publicly advertised. You would need to call and speak to an operator. Once you have been transferred to the person you need then they will be better able to let you know the nearest fax line to their desk or office.
uber, lyft, taxi, family member. If you call 3 days or more to book your appointment then you should not have an issue being picked up. Your area may not be saturated with multiple ride providers so if you are not booking your appointment early enough then there may not be enough time to properly get you a ride. Once you properly schedule a ride logistic care gives a confirmation number for each trip. If you didn't get one then you didn't complete a pickup request properly. Review these steps and make sure first that you are doing your part as the client.
It depends on weather taxes were taken out of your pay check. If it was, you would file as an employee. If not, you would file as self employed to pay your taxes
