I believe all birth certificates are $25 paid by Credit/Debit right now as I just went last week. From what I see, you can save time by going to their website and downloading the Amendment Affidavit to change the name. It would have been free if within the first year of birth and since it is a new name and older than a year, you'll probably just pay $25 as in a new certificate. Hope this helps but I'm not entirely sure. I would definitely check their website. The office on Phoenix Blvd is currently not open to the public but the one across from the courthouse downtown on Pryor is open and the wait was less than 20 minutes tops even from parking across the street.

You'd have to transfer your license to the state of Georgia which I know it's a fee of a few hundred dollars not quite sure of the exact amount and they will give you a 30 day temporary ID which will give you time to acquire all the required documents. You'd do that at the DMV not vital records. If you're not from Georgia you can't get anything from vital records you'd have to go back to your home state to retrieve your birth certificate.
