Usually the office is backed up by a month or three, so paperwork sits and sometimes/usually gets forgotten. Bring the IDs and cards listed below. If you are getting any SNAP bring a bank statement. Forgot to add in, you should expect to be there 3-4 hours minimum, otherwise you may get lucky and finish quickly.

No, I have never actually spoken with anyone on the phone by calling, you can try leaving a message if you have a case worker. But honestly best if you just go there since you are guaranteed to speak to someone
Honestly, go in and talk to a case worker, everything will be so much easier.
They have been behind on their caseload for about a month and a half to two months. I would go in and get it settled right away.
It is on your medical card(technically insurance card) NOT the paper card you get from public aid but the one you get once you pick your insurance company

A new one is sent in the mail. The funds from the list one will transfer to the new card. It will take approximately a week and a half to about two weeks to get the new one in the mail.

Medical through the state (Medicaid) is redetermined every 6 months to a year. To get your application pushed through quickly, I would recommend spending time at your local office and speak to a caseworker. You will need bank statements, pay stubs, and social security card to determine your eligibility.
You can go on the Illinois dhs website to request a replacement card. They don't keep actual cards at the local offices any more. They have to have them mailed to you.
For underemployment you would actually go to the unemployment office on smoke tree lane in North Aurora, or you can apply for What Department of Human services calls TaNF temporary assistance for needy families if you expect it to go 6 months to 1 year
