I would give zero stars if it wasn’t for the wonderful physical therapists there. I felt compelled to write this review, albeit 4 years overdue, as I feel this will give me the closure I need to heal, as I lost my mom at this facility and she would have turned 81 today. My sweet mom was a patient there 4 years ago, receiving rehabilitation after a cauda equina diagnosis, which is a medical emergency and can cause loss of bowel and/ or bladder function. There were a few times that I would visit my mom and she would be sitting in a wet diaper while in her wheelchair. One time there was actual urine on the floor under it. My mom told me that she had been waiting for 45 minutes to get changed. Either the staff didn’t “see" her light on, or wouldn't respond to her because they were busy handing out dinner... To say I was livid was an understatement. Another “ flag” was once an aide walked in my mom’s room to change her, eating a bag of chips, and proceeded to violently vomit in my mom’s toilet afterward. The aide knew her toilet was not working ( as my mom was waiting for it to be fixed for 3 days at that point), but I guess she forgot and flushed it. Needless to say, it never went down and when I told the nurse at the front to have someone clean it out, she told me that she would just have to wait until the morning when cleaning crew came in. I asked the nurse why couldn’t she just clean it for her, and she responded that she didn’t do “ those things.” When I called my sister about it and sent her a picture, she said I should have turned them in to OSHA. Another time a new aide lashed out at my mom and accused her of soiling her diaper “ on purpose,” since she had to change my mom twice in a 45 min period, as if my mom had any control over it. Isn’t this their job? My mom went into cardiac arrest while there- 1 month before she was due to come back home. The facility had a “ patient coordinator “ call me at 11:50 am, on the morning my mom told an aide she couldn’t breathe ( at 10:30 am) while he picked up her breakfast tray. The facility said they left her room for 2 minutes to get an oximeter,, and when a nurse returned to her room, she was unresponsive. The EMS wasn’t called until 11:40. My mom was pronounced dead at Medical City Lewisville at 12:07 pm. When my family and I compared medical records of the hospital and EMS versus Lake Village, there were definitely inconsistencies in the facility’s part, not only in documentation but what was told to us verbally. In fact, the attending doctor in the ER said to us, “ what were you told by Lake Village,?” when he called us into the room. Four years later, and I still feel so much guilt and I am heartbroken at the treatment she received here. If it wasn’t for her losing her insurance coverage if she moved facilities, she would have not stayed there for the 7 months she was there. I feel for those residents there, especially those whose family can’t or don’t visit them often, to be their “ voice.”

I just want to say thank you for caring for my aunt and cousin. My aunt has been a resident for over 10 years. She was so well taken care of. I was born and raised in Lewisville and the facility is probably as old as I am but yet you would never know. The nurses and caregivers kept me completely informed, even in the wee hours of the night lol. I just can’t say thanks enough.

On behalf of my grandmother & our entire family we would like to thank the staff at Lake Village for a phenomenal experience! The care especially was exemplary!! They not only catered to my grandmothers needs, but to our needs as her family as well. I highly recommend this facility to anyone looking at placement for a loved one!!
