Japans pretty strict about covid policies and testing especially during the pandemic. They make no exceptions which I understand since this is their literal policy. I haven’t been able to go to Japan to visit family in years even though I was booked to go in the winter of 2019. My grandma is 98 years old and recently placed in a nursing home. I was supposed to care for her the following year. Unfortunately covid happened and my relatives were able to make other arrangements for her. I came recently for a passport application. Because the visa process seems unusually frustrating. Five stars because it was smooth. It’s a bit of a wait even with a reservation but that applies to any embassy or for example the DMV. The lady at the counter was beyond sweet and so helpful If you understand anything about Japanese culture you know that they’re very firm on following policy no special cases for anything (except maybe the Rich and powerful- both of which I’m not ).

The NY consulate failed us after numerous guarantees that we would have our visas in time. Inefficient, and downright mean, honestly. One would think as Japan’s policies have gotten more arduous to navigate, that they would offer some sense of understanding and/or deliver on promises made to deliver visas in time. But alas they left me and my family completely empty handed, and out much of the cost of the trip based on proximity to travel date.

If you plan on staying in Japan fewer than 90 days, no. If your stay will exceed 90 days, you need a visa.
Visas are required for entry temporarily even when from usual trusted nations. You will also need a guide and travel plan before applying.
Unless you are citizen of USA, you may apply visa in advance. One of my friend is Indian citizen who has green card but had to apply visa before visiting Japan. So it's better call embassy what county's citizen you are and ask if you need visa or not.
If its a doctor perscribed meducation all you need to do is bring a written document on that and most places and countries will give you a pass on it as long as its not over a certain amount. However if uts gokng to be an issue you may want to choose a duffrent spot to vacatikn or travel to

if you stay in japan less than 90 days, you don't need visa as US citizen. The law can be changed in the future so it's good to check the rule before you travel to japan again.
