they now refuse to see me without a face diaper even though I have COPD and literally cant breathe wearing their ineffectual face mask, I told them I actually passed out the one time I wore a mask. they are very hateful and unprofessional, they spew liberal anti American propaganda.

My children and grandchildren as well as my grandchildren have been going here since we moved to Florida 😊 they ( the kids) love doc Brian( Brian Brooks) he's always great with the kids! Makes them feel at ease and comfortable..Wich isn't easy especially for Dr in Dr office..all the staff here are wonderful!! My dad even started going here as his new primary care physician...they also have their own in house pharmacy which makes it easier so you don't have to go all over the place to pick up your prescription.. especially when you have a sick child... Recommend this place to anyone in need of good hometown primary care physician doe yourself and or the kids!!

I 'had' to go to this place because they were the primary on my insurance card. I only went there to get a referral. As I left the office, the physician assistant said he was placing the referral that day. This was 3 weeks ago. I called the facility that was supposed to have my referral 2 weeks later, they said they never got it. I called Azaela Health to find out what happen, and they transferred me to the referral girl, which went straight to voicemail. I left a detailed message like it requested, and one week later I never heard anything back STILL. I called again and asked to speak with the girl, she wasn't there. I called back when she was there, and it went straight to voicemail. This is absolutely ridiculous. Why does it take 3 WEEKS to get a referral faxed over to a doctor's office?? In a normal office this would take an hour. I would never recommend anybody to go to this office unless they HAD TO like I did. The doctors were professional, but the other front desk staff were less than professional. The place was musty inside. Additionally, the survey information you had to fill out when you walk in, was ridiculous! They ask you questions like 'How many times do you have sex', 'Which gender would you like to be called', 'How many life partners have you been with', etc etc.... It didn't even ask what the reason for your visit was. Absurd.

I have never dealt with an issue at this particular office in the past. The staff, especially Brian, are caring and thoughtful. There is one nurse, in particular, who works here who needs to be genuinely reassessed. She has acquired a personal distaste towards me due to the reason that I have challenged her ability to do her job. There have been many attempts to try and have a simple fax sent over to the dean of students office at my university, ACCURATELY. Time and time again, one thing after another went wrong. After leaving a voicemail inquiring about this issue, the same nurse of Ted Soldatos called me back (days later) after closing time. I asked if the fax could be sent over immediately, as this was an urgent issue, and she claimed the office was closed. This led me to wonder why she would be calling me after closing time to speak on the issue; She then explained that nothing could be done about it because they had closed minutes after I had called. My school has a time restraint as to when these excuses can accurately be sent in. Fortunately, after explaining the issue, my school was able to cooperate and ensure that this was not something I could have prevented. However, had it not been for the kind nature of my school staff, I would have had to drop my class. Please consider staff hospitality before hiring!

The drs at Azalea Healthcare are very good, I've been a pt there for yrs and like that its small, local and easy to get into be seen. They handle children and adults regular health problems. You should definitely call to set up to be seen. Hope this helps.

From what I understood they have to refer you to one, depending on your insurance. Azalea has a network of different things like dentistry and women's care. They could always find your one that they trust and recommend
