My visit with the certification center was not a good experience.The Dr.was rude and acted like he was. Lawyer and very Unprofessional. Jim at the front desk was very nice maybe field welcome.

They will provide the medical release form online & you sign digitally, so let your pain clinic know you DID sign a med record release; they have a naturopathic medicine name besides the CCC title so that is what your Dr's office will see for the name.

They dont sell marijuana. That's to get your medical card. They have like wraps and bongs and oils and stuff but not Marijuana

PTSD is a qualifying condition for a mmj card. You do need a diagnosis and/or treatment for the condition with medical records from your doctor within the past 12 months to qualify.
Each state may vary with qualifying conditions. Presently, insomnia would not qualify for an AZ medical marijuana card. More information can be found on the Arizona state website at https://www.azdhs.gov/licensing/medical-marijuana/index.php.
Thanks for asking, Kevin. We will contact you in plenty of time to renew. Renewal will be as easy as possible if we get you in by the anniversary of your last visit with us. Basically, you simply need to bring your ID and mmj card to the clinic to see our doctor to recertify. We will complete your state application and your new mmj card will be mailed to you. Truly, it's a very easy process. We will give you a quick call this week to walk you through it.
I have a chronic pain condition and they usually just focus on your worst symptoms. They are absolutely wonderful! As long as you renew with them annually your Rx will stay in their database! If you do a Google review they have a few different gift options!

Yes it is $300 total without food stamps. With food stamps it is $175. Part has to be on a card so I would call them before you go to gets the deets about the card and cash portion

The doctor will examine you to see if you have a qualifying medical condition. They are great.
If you call and talk to the office they are really good about letting you know what you need to do.
Call and set up in the morning they are pretty quick in and out.
