This is a hellish place. The building itself is so run down and dilapidated I can't believe they house people there. It is depressing just to walk into. The man I know who is in there says the food is horrible; they serve hotdogs without ketchup or mustard; cold scrambled eggs without salt or pepper, and butter is never available. The minute I walked into the place someone told me I would have to wait in line to use the elevator because it only takes one person at a time. There were seven or eight people in wheel chairs waiting in line to use the elevator, so I took the stairs. The next thing I heard was someone saying "I'll reset the elevator!". I would not get on that thing to save my life. I don't know what the allowed occupancy level is in that building but to be inside the place you get the definite feeling that there are too many people crammed into a very small space. If people are not depressed when they enter this place, they certainly will be soon! Do not put your loved ones in this facility!

Felisha, the social worker, the one in charge, she will never fix Problems that appear within the facility because she’s apart of the problem!!SIMPLE TASK such as brushing hair, showers, brushing teeth are not completed, if I didn’t come take care of my mother myself they would let her deteriorate. I’m angry that such people would pick a profession and fail at it, as theses people in theses facility need all the tender, love and care and sadly the workers don’t and can not provide that. I DO NOT recommend sending your loved ones here, I can say this is one of the worst faculties in the state, if not county. The Staff’s attitudes are disgusting ,and they are allowed to threaten, and be disrespectful to family’s members with no discipline or action taken. Managers of the building are just as bad as staff if not worse, no problems are EVER resolved. The whole building needs to be restaffed, especially with the very disrespectful lady’s on the 3rd floor. I have ran into maybe two staff that were helpful, let this review help you and send your loved ones somewhere else more stable where they have better attitudes and care for residents better.

My wife was in Crossroad's for a month for rehab. After reading the reviews I was upset this was the only place that would work with her. She had a tough time adjusting for the first few days but the staff took good care of her. They responded in a timely manor when she needed help. Meds were always on time. Every one of the staff were friendly and willing to do what was needed. I would not hesitate having her go back there for rehab in the future. One LPN (Natasha) stood out as being a very good caring nurse. The only drawback was the food. Good portions but little flavor.

Please let this place be your last option for your loved one!! My Uncle is bed bound; was born with cerebral palsy; always needed a lift to help with transfers and he still has his strong mind!! Wait!!!! Don’t let me forget to mention that he’s too weak to push and pull! Within 3 weeks of being there he’s fallen twice! The first fall was at 10 in the morning and I didn’t get notified until 4:30 that evening! They didn’t send him out to get checked;I did! Luckily nothing was broken and no bleeding! Trisha (DON) right along with David calls me and told me that wasn’t my place to come there and send him out! She also lied and said that they had X-ray equipment there to check him out! They made me feel like I broke a rule! Then I remembered! I’m his POA!!! I have rights!! So yeah we asked questions!! Didn’t get any answer to how long was he on the floor, who was around, why wasn’t we notified right away and etc! Unfortunately! He fell again! He has a roommate that moves around a lot! This time my uncle tells me that he was pushed out of the bed and he mentioned his roommate name and the nurse that was there! Again no answers!! So now they are angry because we are asking questions! We got told to leave and that the police was on the way! He’s in the hospital with an infection! Due to them not cleaning him properly! They never notified the people higher than them ! So we called the State the first time and this time too! The Worst place ever! Staff always outside smoking or sitting in their cars! Nasty attitudes and very disrespectful! I should’ve known something wasn’t good when I saw how the building was ran down!!🤦🏽♀️But yeah we have someone on the outside investigating for us!!! Please take the time to find a good place for your loved ones!!
