Sorry, I don't think this is where you put that request in. This asks other patients questions like "Is there a wheelchair accessible entrance?" or "What type of insurance do they take?", it doesn't go to the office. I would try to log into your Rothman portal to access your records, or try calling the office. Good luck! 😊
