St. Croix Hospice

St. Croix Hospice provides coordinated care throughout the Midwest and works with families and facilities frequently to prepare our local teams to care for a patient once they arrive at their new home. For example, if a patient is discharging from a large metropolitan hospital in one state, but will need hospice care when they move to a home or facility in another state.

St. Croix Hospice works directly with hospitals, nursing homes and assisted living facilities to coordinate a seamless transition to hospice care in partnership with the facility care team.

St. Croix Hospice exclusively provides expert, compassionate hospice care, including a variety of specialty care programs to meet patients’ specific needs.

You can make or adjust your care goals while receiving hospice care.

Hospice care also includes social, emotional and spiritual support for patients and their loved ones—including access to additional community resources.

You may continue to see your primary doctor for as long as you would like; hospice works in partnership with primary care physicians.

Patients with any life-limiting illness or terminal diagnosis can benefit from hospice services.

St. Croix Hospice care is paid for by Medicare, Medicaid and most private insurance.

Anyone with a life expectancy of six months or less may begin hospice care. Patients who live longer than six months may continue to be eligible.

Hospice is not a place, it is a service. Hospice care can be provided wherever patients call home including assisted living facilities, nursing homes and private residences.
