IF you have a Polish passport, you have to apply for Polish citizenship with the Polish passport as proof that you are from Poland. If you don't have a Polish passport, you have to send documentation to the Polish Consulate in D.C. and prove you you are a former Polish citizen. Then,you can apply for a Polish passport and travel to Poland as a Polish citizen or go to Poland on your American passport and take proof of Polish citizenship with you to Poland and apply in Poland.

Yes, you'll have to bring your birth certificate and any other official paperwork. https://www.gov.pl/web/gov/uzyskaj-paszport-usluga-dla-osoby-doroslej is a good place to start checking the requirements. Getting an appointment is difficult, check the availability frequently here: https://secure.e-konsulat.gov.pl/Default.aspx
Definitely call the consular emergency line and explain the situation, they might be able to issue a temporary Polish passport in a week or so. Otherwise, contact the Polish border patrol and see if they'll let her in on an American passport if she has one; in my experience they have been very understanding. Just try to get a relative in Poland to make the call for you to save some money on a potentially long international call.
