A referral is preferred but if you are having difficulties obtaining a referral our intake coordinators will help you to obtain the information we need to schedule your first visit. Please keep in mind that some insurance plans require a referral from a primary care physician to see a specialist. Hope this helps. Don’t hesitate to call us if you need any assistance.
We are currently seeing patients in Lake City from 8am to 5pm on Monday, Tuesday and Thursday. We are available by phone 8am to 5pm Monday thru Thursday and 8am to 4pm on Fridays.
