Lebanon is the worst customer service I have ever witnessed, after filling every thing out giving more info then needed, they still don’t put your applications thru! I thought they worked for us, no way they think we work for them and oh yea the rep hung up on me after I waited twice for an hour!

They are not fair, accurate, or timely. Applied for benefits over a year and half ago online, they sent a request to complete an additional form online, however that form could not be found.I called and spoke two agentsThey said where it could be found on their website, all info they gave was inaccurate. Was not given sufficient time to complete those forms online.and got denied. After I appealed, they stated that they sent the made me feel that I was not telling the truth. Just because someone's working does not mean they are not disabled. Calling out of work often struggling to go to work, struggling with pain, and working my entire life since I was 14 years old and now almost 50 years old and I'm not able to even get a respectful response back is ridiculous. They denied an appeal a year later. That letter was rude and made it seem as if I was trying to lie for benefits that I didn't need because I was working working a gregarious job at ....., should not disqualify me the way they made it so

There is no real parking...you have to park blocks away. All parking nearby is for permits only...they do not consider the elderly folks that come into the establishment.

I'm not sure how much you can make but I do know after you go past a certain amount for every dollar you make they take $1 away
Thank You Marci Howell. I took a chance and went out there early last Wednesday and fortunately I was there for 8:20. Turns out they didn't open until 9am. I like the way they are doing things since opening. I was in and out in no time at all.
I don't have an answer but have reported it to our federal legislators that it is an ongoing problem. Supposedly there are resources online as well but I haven't tried that route.
I was there recently there is only one person working in the office they will not let you in unless you have an appointment it took days to get threw on the phone best thing to do is keep trying good luck
Unfortunately no. You have to call the 800 number and set up an appointment for replacement social security cards as well.

Everything you can think of to gather that proves your identity (drivers license, birth certificate, address proof, etc etc take everything you can find even if it's not needed!!)

Usually you lose all benefits from the state whether it be disability, EBT, Medicaid, etc if you're incarcerated and then you have to just reapply when you get out.

The only way u can change ur name is u got married. Take ur marriage certificate.

Go inside the office and tell them u want to change ur address. Make sure u take proof of ur new address.
