Just a heads up. They close at 4 even though it says 5. They shut down their system at 4. They can’t/won’t print out anything after 4. So that is super frustrating and not noted anywhere in the hours here.

where are you doing the senior citizen Covid vaccinations going to be given?
No, you stay on the notification list until you have successfully booked an appointment. If you have reached out to us by telephone and cannot access email, someone will be contacting you within two weeks to assist you in making an appointment.
the building is very clean and no one is sick idk why your app is showing that but ok
Yes, it takes about 30 minutes if you have all of the information filled out and I.D. See http://www.webermorganhealth.org/administration/vital-records/ for more information.
We use a third-party vendor for online orders that also provides services in other states. Not sure Texas is one of them but it doesn't hurt to check. It is more expensive through this service than coming into our offices but it may help you with an out of state order. https://www.vitalchek.com/
If you were vaccinated within the past 7 years and your provider entered it into the database, we might be able to tell you. Give our Immunization Clinic a call, 801-399-7250. Thanks!
Contact the health department in the county of the state where you were born. Or, we use a third-party vedor called VitalChek https://www.vitalchek.com/birth-certificates that may have information on the state you are seeking.
It's $25 and here's the info on when it's taught and how to register. If it's easier, you can also register over the phone by calling 801-399-7160. http://webermorganhealth.org/environmental-health-services/food-services/food-handler/
You can book an appointment at one of our vaccine events on vaccinate.utah.gov. There are also many pharmacies in our area that are now offering it.
Try and just go in
We will be working off the waitlist established Jan. 8 when Gov. Cox announced we would begin serving people age 70 and older.
You will receive an email or phone call at the address or phone number you verified when you checked in for your first dose. It will outline how and when to make your appointment for your second dose.
I guess it depends on what you're trying to do. More than likely you will have to contact them to schedule an appointment.
Hi Janet, we are only receiving small shipments of vaccine at this time. As the shipments grow, we will be able to offer more appointments and one day the vaccine will be plentiful and you can get it at doctor's offices and corner pharmacies. That's a few months off, however. Thanks for the question!
Go to Weber county's health website Select the little black Box. Self explanatory. Easy
Now call for an appointment
how to get in to health department to get my shot i am 74 year and wife to get appiontment
The governor announced that those 70 and over could start receiving the COVID-19 vaccine on Monday January 18, 2021 (I realize that this is the MLK holiday). I need more information on how I can sign up, and where I will get the series of shots?

We are working with the state of Utah to establish a contractor to perform this in Weber and Morgan counties. It will be similar to the way they did the long-term care facilities.
I believe so
Hello, it's probably best if you come in for privacy reasons but you could call our clinic, 801-399-7250 for more information.
Yes, you can make an apppointment by calling 801-399-7250 and find more information at http://www.webermorganhealth.org/clinical-nursing-services/tb/
Hello, our Environmental Health Division will have more information on that topic than we do. Please contact 801-399-760 and you may also need to contact Ogden City.
Taught every week, only on Tuesdays. We offer in-person training for food handling permits. If you work in the food industry in Utah, you need to pass this test. Our classes are offered to residents of Weber and Morgan counties. The permits we issue will be good throughout the state of Utah. We believe that proper food handling training helps protect public health in our community.
you would have to contact the Weber Morgan health department and ask them
Just call Environmental Health at the Health Department. Give them your information and they will verify that it is still valid. If it is, it is about $5 to replace it.
No but it is helpful so you don't have to wait.
I think you would have to go in and get it your self
If you were born in Utah after 1932, most likely can help you. Please call come in or call, 801-399-7132.
Hi, thanks for your question. It is against the Utah Indoor Clean Air Act to smoke or sample cigarettes or electronic nicotine devices inside smoke shops.

Yes. But only certain individuals can request a copy of someone else's birth certificate, usually immediate family, legal guardians, and/or spouse. There is a fee which I believe is about 10 dollars. The office is located in the south end of the main floor.
Yes to tetanus shots and we have a program that offers vouchers for free mammograms.
