I went into get immunization's, and the nurse that administered the vaccines was terrific! After i was done and waiting for the front desk to enter and print my info. I waited for around a half hour, and nobody was doing anything besides talking. I went up to see what was taking so long, and they could not find my paperwork, and then when they did the girl CARLY was the one who started it and left it unfinished so she could go get food from her mommy that wanted to surprise her for lunch. Horrible and lazy people!!!! I will NOT be going back for my my second doses!

You need to call the Health Department and ask for the Health inspector so you can make a complaint
Birth and death certificates can be obtained in Vital Records, on the first floor of the health department.

If you visit the immunization clinic on the first floor of the health department building, they can give you a new card. Just make sure to bring ID!
$15 for the tuberculin skin test.
