The complex that we live in it is so nasty people in a complex have called at least 50 times no one has ever came out and looked at . Some of the apartments don't have smoke detectors there are no fire extinguishers but none has ever came out it's bugged infested the pool has not been cleaned in 2 years and a lot more

Don't trust their std clinic. I went there for Chlamydia bcus my boyfriend had it from his previous relationship and they lied my tests was negative but I also had all of the symptoms. The nurse was very nervous and just gave me meds. So what's the point of the test?

Yes, you receive the certificate the same day if you visit our office in person. You can also order certificates online through our website and have them mailed either overnight through express delivery or in 7 - 10 business days through regular mail.

Yes, you can purchase both "Abstract Certificates", and "Live Certificates" from our Vital Statistics Office located on the 1st floor of the Reibold Building (117 South Main Street Dayton). Feel free to also give us a call at 937-496-3117.
PHDMC does not offer appointments specifically for PAP screening / birth control prescriptions, but if a patient has an appointment for STD testing and then needs PAP testing and/or birth control, we would provide those services during their appointment. Please also note, our prescription would only be for a three month supply and the patient would be required to follow up with a gynecologist. The “Center for Women's Health — Five Rivers Health Centers” on Wyoming street does offer appointments specifically for PAP screening / birth control prescriptions. They can be contacted @ 937-208-2007.

Hi Martha, We had approximately 450 doses of vaccine available for registration that began Friday at 8:30 am via our website or phone line. Due to the large number of people seeking to register, all the doses were accounted for very quickly.
