I really enjoyed myself at the mid city ballroom. I got to see one of my favorite bands there so my opinion may show bias. The old churchy vibes definitely helped. At one point the singer of Every time I die mentioned how rad the stage and background were. Fair warning it gets really hot in there when it's at capacity for a sold out show. Oh and the vinyl record area is stacked. I recommend checking it out if that's your thing. They had wheelchair accessible entrance and restroom and such.

I would go to the actual place. Bring a demo and a complete listing of all your set music.

No. Its bring your own drinks, alcohol or otherwise.
Bring ID and present at door. They check your name on a list on tablets and give armbands
I have no idea what the Fire Marshall would say but I'm guessing 75-100 in the ballroom without room to dance.
Mid City is a music venue that books a variety of genre. So far I have not seen a kids-specific act. On the other hand I haven't seen an X rated act either. It's a b.y.o.b. kind of place but I haven't seen anyone get out of hand, with the exception of the woman who would not turn her phone off.
