Call 800 922 0204, you will be on hold for a while ,unless you choose to get a call back. Listen to prompts for dept you need. Best time I have found us Tuesday thru Thursday after 1pm EDT.
the closest verizon fios retail store is located on Stewart avenue just outside of roosevelt field mall next to FedEx.
