Just make an appointment with the doctor on their website (in their description on maps). After the doc finds you in need and the attendant sets up your Florida registry account, you then pay $75 To the state and ~$190 to them. ~14 days later you have a card. Bring any papers to prove your case (doesn't hurt to bring doctor proof). Good luck.
