20/10 reccomend The Chop House! This was our first time and we had the best service, Rob was our server he was incredible. The atmosphere was amazing especially for Toledo, the food was the best food I’ve ever tried. My husband got the Waygu Ribeye and I got the cowboy steak, I cannot say enough great things!

I've only done reservations. But I believe you can walk in most times.
It's expensive but go around 5. You can order cheaper appetizers until 6pm
Yes. There is valet parking. It's a high end restaurant. It has amenities to match.
It wasn't required last time I was there about 2 weeks ago. But masks are recommended if you're not vaccinated.
Mask before sitting, not all tables are occupied, servers/staff wear masks, all guests have temperature taken before being seated.

I have never seen anyone who wasn't in a dress shirt. It's a classy restaurant. The waiters are very well dressed and classy looking. Dress accordingly.
Semi formal is fine ; though you won't feel out of place with a dress shirt and jeans. It's the kind of place where business people will want to go. There is even Valet parking, if you want. It is definitely an upscale restaurant, with really good attentive service.
You can use open table to set up a reservation.
Scallops, $35, plus any sides you order. Salads are extra, not included as said below. Food and atmosphere are great.
