Great Physical Therapy place! The facility is spotlessly clean, open and airy with lots of sunlight. Modern equipment. Plenty of free parking. The staff is excellent. Lead Physical Therapist (PT) Dave, PT Stephanie, and Administrator Pam are professional and very friendly. I feel like I'm family when I'm there. While they are indeed friendly, don't expect to just have Tea and Crumpets when you're there. PT Dave is a results-focused Therapist, and he will develop a comprehensive program to address your particular issue. When I leave my session, I know I have had a thorough PT session. I'm already getting relief from my chronic pain, and this within just two weeks. Highly recommended

Best place ever, Dave is great and Pam is wonderful when you first walk in she makes you feel welcome and Dave is great at what he does very knowledgeable

Highly recommended! Dave was awesome! He always took the time to explain everything so I could understand his treatment plan. Pam made scheduling appointments so easy and I was always able to get an appointment that fit with my busy schedule.

Each patient's diagnosis is different, so together with your referring physician, your therapist will develop a plan of care that is right for you. Your plan of care and number of visits will be determined during your first visit and explained to you by your physical therapist.

What should I bring to my first appointment?

Please have your prescription and insurance card available when you call. This will allow us to verify your insurance benefits and provide you with an estimate of coverage. To bring on your first visit: A prescription (if needed) for physical therapy from your referring physician, ARNP, or PA. Insurance information (we will make a copy of your card). Print out the appropriate patient forms, fill them out, and bring them with you. In case of an automobile accident or worker's compensation claim, also bring any case manager or insurance adjuster contact names, phone numbers, and claim number. Please arrive 15 minutes early to complete any remaining paperwork, so you can maximize your time in your evaluation. Please wear comfortable clothing that allows easy access to your problem areas.
