It depends on the benefits of your insurance policy and they do have cash pay rates as well depending on what you need done

You called on veterans day most doctors offices are closed in a holiday. Also check in is by phone also that maybe a reason you don't get anybody because staff is busy assisting current patients at peak times
I know they take Medicare. I'm not sure about Medicaid. The best thing to do is call and ask them.
I did take Cigna and made the appointment without a referral. However, it would depend upon your particular coverage. To be safe, I would call Cigna and ask.
