So what can i truly say. Because of Jeremy and Ms Connie I feel i can continue living. Listening to Jeremy share his story of how he overcame his trauma to fight to live gave me strength. I literally was going to end it after my appointment. I was in a place of how can i live after this. But he was patient. He was calm. He didn’t try to throw information at me. He allowed me to cry, he listened, and that was what i needed. Getting the warm motherly hug and laughter from me Connie made me feel at ease. She was a ray of sunshine. And let’s not forget the treats lol. Dr Charles was very attentive. He answered all my questions of my choices on treatment. But I’m glad i was referred to affinity. I have a family away from family.

Staff is on top of it. I had an appointment at the end of September, and had an unexpected trip come up and needed to be seen earlier. Cancellations are rare and they are extremely booked out ( I booked this apt in August) so I call first thing and Catherine was super cheerful and extremely helpful. Lucky, there was an opening and she got me in right away. I come in and Dr. Jamie Cosby was an absolute pleasure and so helpful. I wish I could remember her sweet nurses name, but man, was it a breeze. A little bit of a wait, but hey, it was worth it. Thanks team for working hard for your clients!

HORRIBLE!! My past few experiences with this place has been absolutely ridiculous. At my last appointment, they told me they must have “forgot” to put me in for that day when I showed the woman at the front desk after showing my appointment card. Had to go back the next day (not close to my house). I placed a refill for my prescription last week, and Walmart said they are waiting for approval from the dr. Well, here it is almost a WEEK LATER and I am without my medication. When I called this morning, the nurses inbox was full and couldn’t leave a message. Had to call back to speak with someone at the front desk. She informed me there are “no notes on my file”. Do not waste your time at this place. The wait times are another story, I am looking so forward to going somewhere else.

Affinity does have a dental clinic and extractions can be done. We do not require insurance but fees are based on income. Our dental fee scale is on our website with our medical fee scale. For an emergency you wouldn't go to a primary care provider. You would go straight to the emergency room. There are obviously some important details that you didn't provide in your question.

Thank you for reaching out. Yes we do provide mental health services and we do accept Medicaid. We have counselors on staff and will be adding a mental health clinician in January to assist with medication management.

Hello Tasha. Our doctors and nurse practitioners determine prescriptions based on the diagnosis and health history of the patient. You would need to schedule an appointment to talk with our staff about your health and health needs. Thank you for your question. If we see someone with a kidney stone, we would probably refer them to a urologist. For a suspected kidney stone, one may need to go to the emergency department.
Affinity does not currently have a psychiatrist on staff. We do have a consulting Psychiatrist who assists our providers and we help our patients get connected to psychiatry.

Yes you can see a doctor. bring your blood pressure sheet or something with your numbers that you cuz they will want to know how your blood pressure is everyday and if you taking it constantly and yes even if you are waiting you will have to pay a co-pay though and it's 5 usually like. you are going to take some Labs the same day

Yes we are able to take walk ins most of the time but not between 12-1:00 or between 4:00-5:00. You may have to wait to be worked in to the schedule but we do our best to see all walk ins. We are planning to be open today assuming the weather doesn’t get too bad.
To be a patient, you can call to schedule an appointment and we will get your information by phone. It’s also possible to become a new patient as a walk in. We collect proof of income and insurance if you have it. It’s a fairly simple process. We welcome new patients!
