You need the registration for both cars but not the cars themselves. You need your registration, your license, proof of insurance and the form filled out.
I think you will have to make an application for a replacement title for the original. Once the replacement is received, you can request a change in the title. Visit the state DMV in whichever state the vehicle was originally titled in to see how to request a replacement. Most have this info online.
