You will need to visit our website @ www.rcprojectaccess.org, go to the "New Patients" section and download the New Patient Enrollment package, fill out the applicable forms and return them to us by mail or you may drop it through the mail slot of our office.
Any doctor's office that accepts it is found through Project Access. For example, I needed a dermatologist and Medical Associates Plus submitted to Project Access a referral and they found one for me to go to.
