Great service up until my primary retired, then they failed to assign me a new one, then COVID hit and now it's a run around with double bookings and excuses of " we're not taking new patients " , unfortunately I've been a patient there for over thirty years , so yeah not on my list of great places to go for treatment

Yes. Lifespan lab is located lower level in the back of the building
I'm pretty sure they have an x-ray machine but you need to go to the hospital for MRI and CT scan
They can order the bloodwork or send out cultures to have the necessary tests done, so yes.
