From my experience if you need paperwork completed by a deadline and you have to get clearance from your primary doctor first don't wait till the last minute they don't fill it out right away. Once you get the paperwork back from your primary and take it back to exam professionals they will take forever to get it back to you. They only have one doctor and they are constantly busy.
