I have been calling every week. for The month of July, and daily in August with messages and emails to my worker then her supervisor yet still no response i understand they are busy..But not to answer or email back for 2 months? Very disappointed!!! especially when supervisor says she would call before end of day 4 days ago.

You can probably do that. But if I were you I would go in person because now a days it is not safe to mail your personal info out. Is because someone can just steal it out of the mail. But that is up to you.

Call the toll free number and complete their interview and they will schedule an appointment. Complete all the paperwork before the appointment and bring any supporting documents with you. There is also an online process that you can complete and an online guide, as well.
