Probably not. We provide care without a physician referral for patients with most insurance carriers. Medicare, Tricare and Medicaid require prior authorization. If you need help with a referral, please contact our team.

A member of our team -- either a licensed Therapist or licensed Assistant -- will oversee your evaluation and therapy.

Yes. If your child is under the age of 18, we ask that parents accompany them for their initial evaluation.

Yes. To support patient flow and for the safety of our visitors, our team asks that family members remain in the waiting area while you are seen. If you are bringing children under the age of seven, please bring another adult with you to your appointment to supervise them.

Please wear loose and comfortable clothing. This helps our providers easily access and evaluate the area of your body that needs treatment. To protect our patients with allergies, please do not wear perfumes, colognes or scented lotions.

Please bring all insurance cards, a photo ID and a list of all medications you are taking to your initial evaluation. If your insurance coverage changes at any time, please be sure to inform our team.

Please let us know before your appointment if you need to reschedule or if you’re running late. Our team will work with you to find a date and time fits your schedule.

Our team asks that you arrive 10 minutes early to your return appointment, in case we need you to complete any paperwork or ask you any questions.

Our team asks that you arrive 30 minutes before your scheduled appointment so you can complete appropriate paperwork. To support patient flow and help our providers see all scheduled patients, our team may reschedule your initial evaluation if you arrive more than 12 minutes late to your appointment.
