I mailed my application, a copy of my driver's license and a money order for 20.00 in early December of 2021. I haven't received the Birth Certificate I applied for and it is now March 2022! I have left a message on the office voice mail at least 3 times as well as emailing Pat Clark.... so far.... no response. VERY disappointing.

The service was quick and pleasant was my experience. There should be a note somewhere that front doors are locked from outside and would of been nice to receive a call back after leaving a message but read and followed the instructions for certificate and was in an out within half hour.

This place has lost my documents after my name change. Now I can’t renew my passport and nobody picks up the phone at this place. New Haven is a joke

If you was married in new haven ..you have to get it in new have. .. because you got married in new haven is always going to be in new haven yes you're parents name and that's it.
Go to the office if you're local. My husband and I were in and out in 10 minutes yesterday
@Suga All you have to bring is 20$ they have a atm in the building, if you need to get it. Once you get in the office, they have forms for you to fill out. Once done, you just bring it to the clerk and they'll give to you there
