Yourself, State ID or proof of residency, medical records if you have them, payment for the visit, and a credit card to process the State ID card. If you don't have copies of your medical records we will request them for you.
Hi Anne - The initial office visit is $250 or $200 for Veterans. Follow up visits (required at 4 months and every 6 months) are $150 each. Additionally the state requires a $75 card fee once per year.
