Highly recommend art and Andy they did great and made sure to work with each of us individually on the proper way to hold a firearm. Will definitely recommend to anyone who asks me where to go.

The following is my opinion and is not intended to be "legal advice". First, you need to contact the law enforcement or court that revoked your permit in 2015 and seek reinstatement. The Certificate you mentioned is no longer valid (3-year limit) so it can not be used to apply for a new permit. Since you do not have the original issued permit, you can not renew the CHL license. If your permit (actual card) can not or will not be returned because it may be expired, you will have to comply with the current law requirements and complete the CHL course again to apply for a new CHL license. In your question, you did not state why the permit was revoked, which may be a major factor. You may need to obtain the services of a lawyer to assist you.
