You can swing by the front desk and they will give you one. You can also have your doctor's office do a request for information.

Hello, I think that this type of question can be answered better if you call the laboratory. They have people who speak Spanish and could help you better.
Really baddddd .... i came yesterday 8/12/2019 for a 9:30 appointment and waited 2 hours for a mammograam ... I came today 8/13/201 for a Citi scan appointment was 4 3:45, came at 2:50 and is 4:25 n still here and there still 2 more people....
