Our mother was placed there after experiencing a stroke on 2/7 and moved from the hospital to Windsor on 2/12. On the fifth day we participated on a phone conference call to discuss her “ baseline” goals with four staff members. She was to receive physical, occupational and speech therapy. Five days later on a Sunday the Occupational therapist introduced herself and explained she would be working directly with our mother. The Next Day I was informed by “Gary” the case manager that she had completed all her baseline goals and would be released Wednesday 3/24. My mother couldn’t even sit up on her own! She was unable to get out the bed without assistance, no toilet use and could not walk 3-5 steps!! My only recourse they stated was to appeal their decision. I went through the appeal process etc and yet in 3/24 was told by Gary, we had been denied. After having to remove my mother on 2/25 , my mother was not and to this day is unable to sit up on her own, get up and/ or even walk 3-5 steps. On 2/26 , The attending physician Tan Tran and Gary mailed their detailed explanation. It is false information at its finest. I never received the “notice of the decision on my appeal” nor were phone calls returned the following week. Beware also!! On several issues they are not Forthcoming: example wheelchair rental, occupational personnel claiming dr status, Gary case manager making up information. Honestly the hard working staff on the floor and including Angelica and George were honest, helpful and professional. Note: According to the Levantine LLC report I received later states “ Documentation shows patient walked several steps. However, was not able to find this documentation in the physical therapy notes” Be Aware!!!! My mother is still suffering and in need of Assistance.... yet They received their Insurance money!!

Older, but nice place & good staff. They are genuine people, that really care about other people. But, like anyone who has extremely needy people to care for, the the staff too can get frayed edges.

Oh i have so much to say but of one thing im sure all of this persons working here have great wonderful heart anb y can only say thanks and have 4 more weeks too on my knee therapie

When admitting a patient, what "supplies" or personal things should the family bring?

We supply all needed personal items. However, a few changes of clothing and something to personalize their stay such as a family picture are always encouraged. Please don't bring anything of significant sentimental or financial value.
