Andrea is the heart and soul of this place! She is educated, informative and knows what each patient needs. I send everyone I know straight to Liberate and tell them to ask for her by name..

Do you have to recertified every 180 days

Susie Holder | Sep 16, 2018
Liberate Physician Centers | Sep 16, 2019

There are 2 important timelines to keep track of. The 1st is the State Medical Marijuana Card. This card is issued by the state and allows you to possess medical marijuana products and is good for 1. The State of Florida charges $75 per year for this card. The 2nd timeline is the physician certification / recommendation. This (along with the State Card) allows you to purchase medical marijuana products in the State of Florida. Currently, the State of Florida requires a maximum of 210 days (seven months) between physician visits to keep your recommendation / certification for medical cannabis. Our office charges $250 for the initial certification the $150 for each 7 month re-certification. There is no yearly $250 fee to our office

How much does it cost to get your card

LINDA PISCITELLO | Sep 15, 2020
Liberate Physician Centers | Sep 15, 2020

Our current pricing is $150 for the Doctor appointment (for both new and returning patients) and then you do have to pay the yearly $75 state fee. We do have a referral program that can also save you some $$ off of our fees!

Do you honor Medical Marijuana cards from other states such as California?

dargroves | Sep 15, 2020
Liberate Physician Centers | Sep 15, 2020

The state of Florida requires you to have a qualifying medical condition and be seen by a certifying doctor. This is what we do at our office. After you have your Florida card, you can then make purchases at any Florida dispensary. Unfortunately, Florida laws do not allow dispensaries to sell products unless you have a Florida card.

Would i need my medical records? Most are in Fawcette & some in SMH. Got few at a couple of specialist offices too! Or can I just come for an evaluation. Numours injuries & surgeries & constantly in pain 😢

Dawn Ventura | Sep 16, 2019
Liberate Physician Centers | Sep 16, 2019

Since the marijuana program in Florida is a medical program, we do need to have some sort of documentation that you have a chronic, debilitating medical condition. If you have records already, great! Bring them to your appointment. If not, we are happy to request records from any of your other providers.

As long as you bring your medical records from the VA to your appointment, those will suffice. I am a Veteran and all I did was bring my printed record, which I printed out from the Veterans portal https://www.myhealth.va.gov/mhv-portal-web/home Hit me up on twitter and I'll show you. @shrinkbitch for real, I know the handle is off-putting LOL
