This office is terrible. I waited on hold for over a hour and a half and was told to mail my paperwork certified mail and information. It was sent back to me (even though the address was correct) and I called the office again and they said to drop it off. I go to drop it off and they say they can't take it and I need to make an appointment which they already told me I could not do because me changing my last name was not considered an important enough reason for an appointment. This office has wasted so much of my time and their lack of professionalism has been unbelievable.

Good Naomy by phone it is difficult to connect if you go in person to the office of 1251 nw 36 st you knock on the door they attend you in Spanish it is very possible that they will give you the form you need and they will guide you sometimes you have to wait at the door for it they inhabit

No appointment you have to go very early. If you go to the one of the 57 ave and blue lagoon it is much better. The 36th and the 14th is to be the whole day BUT if you go to 3:00 in the afternoon it is much better because as they are at 4 you have to attend when you take a number.
Once in any social security office you grt a nunber pertaining to your issue. They shoukd be able to help. The cost honestly I do not know.
