My mom died from an infection that went untreated for ELEVEN days. Despite having knowledge that she required immediate treatment for any infection. Her first urine was deemed “old” by the lab when they received it. I’ve never been told what happened to her urine. They didn’t have any straight catheter kits in the entire facility. They neglected to contact Hospice immediately following her death. They rarely had the correct size diapers for her. She wasn’t able to get out of bed for the last two months of her life due to the lack of staff and also her specialized chair disappeared from outside her room in April. The Administrator was going to investigate. I found it in an equipment room on her unit in May. They still didn’t get her out of bed until two days before she died and that was only because I complained during a care planning meeting. I contacted the Administrator, Director Of Nursing, Assistant Director of nursing, Doctor, Nurse practitioners, and the corporate offices in CT for Athena Healthcare during the last two weeks of her life begging for help to no avail. She lived on Hopkins. This unit routinely only has one nurse and one CNA working for 25 patients with behavioral issues.

The Ambassador program is very essential to me and to the facility as a whole. For example if I have questions they usually have the answers but if they don’t, they make sure they find the answers for me. Mirian and Liz come into my room with such positivity and are always receptive of what it is I am interested in doing that day. Mirian is always so bubbly and always will to dance to samba music and to talk about Brazil. Liz on the otherhand is always so willing to talk about music and movies and google what we both cannot remember at times. If my clothes need to be labeled they are always good about doing that for me, if I need a snack they will get it for me and if I just need a laugh they are there for me, I am always happy when they come in. I am very thankful for this wonderful program.

My sister has been a resident for 9 months now. It was a tough but necessary transition in to permanent homing. After many years of health issues and her having been in just about all of the local facilities for various lengths of time, this has been the place that has offered the best care in many ways. The challenges of operating during Covid have been many and we all know the burdens on every facility in the healthcare business. PRHCC did a great job maintaining control of the health of clients and staff during this difficult time. The Ambassador program has been a wonderful bonus. Staff in this program engage clients in various activities and spend quality time socializing. For my recent birthday my sister prearranged a nail painting session for both of us with Liz and we had a wonderful time sharing family stories and laughs. I feel that my sister is in a safe and caring environment.

Hello, We post all open positions on our website, and they get updated on a weekly basis. Please check our website: PlymouthRehab.com

I’m concerned and wonder how many times a week one should receive PT and OT. Does it depend on insurance and how about showers. Once a week doesn’t seem right.

PT should be 2-3x/week. Speak with your loved ones case manager/social worker.
