You can get another one at the office. A guard will meet you outside then a worker will give you some paperwork to fill out, she'll take it back inside and in a short time you should have a replacement card. At least that's how it worked for me. Good luck.
Case are not assigned to workers anymore and all calls go to the 877 call center number. You can visit the office and set up an appointment to talk to a worker if you like.

Log into C4Yourself.com and report the appropriate changes. The form QR7 has been obsolete for some time; replaced by the SAR7 which cannot be completed online.

Yes if you call or stop by your local Social Service office they will assist you in completing all necessary steps needed within the application. It can get denied because 1 box was not checked or answered so please reach out to the Social Services Office for more information. Best of luck to you and wish we could help further.

If you already have Medi-Cal or once you are approved for it, call or visit your county In-Home Supportive Services (IHSS) office to complete an IHSS application. Once IHSS gets the application, a caseworker will be assigned to do an in-home needs assessment as part of the application process. Your caseworker will come to your home to complete the needs assessment: thatβs an interview to decide if you qualify for IHSS. The caseworker will ask you a series of questions about your mental and physical abilities and limitations; figure out the tasks you can perform safely on your own and the ones you might need help with; and evaluate your living and social situation. After the needs assessment, IHSS will contact you to let you know if you have been approved or denied. If you are approved, you will be notified of the services and number of hours authorized.
