Polygraph never charges per question. Price is for a complete test, that allows UP TO four questions written by the client. Sometimes an hour away from your city will be polygraph tests for half the price of testing in your city. Lowest found in CA by searching is $139 while some areas charge $595 for the same test (the test scoring software doesn't know how much you paid).
Any examiner can give you a solid price quote if you know exactly what you want by saying (like) "I think my spouse may have cheated on me so there are three questions I would like testing on". The chart collection portion of the test is essentially three 5-minute question-answering sessions that include the client questions. If you prepare your questions at home, the time (and cost) lowers greatly. Expect to pay $150 to $400, the examiner sets their own price.
Most polygraph exams follow the Federal maximum of 4 client/relevant questions. Price? Nearby typically costs the most, $300 to $500; in other nearby counties, $149 to $225 for the same test. Test near home if short on time but have extra money. Travel to save a lot of money if you have time.
