My father just left this facility after receiving therapy for back surgery. He loved both the nurses and therapists. The facility was great and he even raved about the food. We would definitely recommend Port Charlotte Rehab.

My mom fractured her pelvis in a fall and her physician suggested a rehab facility for a short period before taking her into our home. I chose Port Charlotte Rehab because it had 4 stars on the web and it’s close to our home. What a mistake! I’ve never seen so many unhappy employees in one place! No smiles, friendly greetings and anytime you ask one of them about something, I was met with a blank stare and a shoulder shrug, as if they didn’t know or care. I can’t believe these people were hired to care for our most vulnerable loved ones. Mom was starting to settle in and determined to do what she had to so she could be released to us. I left her on a Monday night and she was in good spirits and doing fine. I got a call from the facility at 5:00 am Tuesday that she had passed, related to her heart condition, supposedly. It’s been 2 weeks now and not a word from anyone at this facility; no condolences, explanation, nothing! Shame on these people for their uncaring and cold attitudes. They are extremely understaffed, and the people they hire do not care about their patients. I now live with the guilt and regret for sending my beautiful, precious mama to this snake pit!

The level of neglect at this facility is awful. My grandmother had to stay for 2 weeks and they neglected her wound so badly that she had sepsis upon her release. We are terrified of what might happen now, because of how harmful that time was for her. She was sent there to rebuild her strength enough to walk after being in bed for 2 weeks with Covid, and she literally came out of this facility in worse shape than she went in. I also had the experience of being on the phone with my grandma as she called for help for 10 minutes straight because she needed help getting to the bathroom, and no one came to help her. I had to get off the phone with her and call the front desk for them to send someone in. I don’t know if the caregivers don’t care, or if it is just set up in such a way that they have too many patients, but regardless of where the problem is coming from this type of neglect is inexcusable. If you have no choice but to send a loved one there, please know that you will need to stay on top of them to get even the most basic level of care.

Port Charlotte Rehab was a lifesaver for my family. My mother in law had hip surgery during the holidays and we needed a place that we could still enjoy the holidays and make memories while getting her rehab accomplished. The rooms were great and clean. Cassandra was awesome at getting us in and taken care of, the transfer from Sarasota Memorial was painless. My Mother in law enjoyed every meal and all the staff was so kind and polite. We had a hard time getting her to leave but we know that if needed again, where she will be staying. Thanks so much PC Rehab!!!

In the SNF section we (family) were allowed to visit at any time. The doors were locked at night, but if there was a need to see my MIL, they would come open the door for us.

If the patient is not Covid positive then the standard practice will be in place. They will get tested as needed and they are not under isolation during their stay. If they are Covid positive we will coordinate with dialysis center and have special transportation.

Yes. You would just alert the case manager at your current facility. Tell them that you would like a transfer to another facility so they may send clinical information to make sure that other facility can clinically care for that patient and they have bed availability.
