I give them no stars. They are the worst place in Polk County. My friends dad was there and they had to take him out themselves because the place was dirty, he had been sitting in the same diaper all day, his butt was raw, the pad under him had rings of urine that had dried. They also have CNA's that are only there one day a week so if you ask them any question about the patient, they will all say "I don't know. I was not here yesterday". Well what about notes about how he is doing? "There are no notes and I was not here yesterday." Do not let your love one go there.

I would like to take a moment to give huge praises to Tayana Carter (RN) she is such a blessing to residents and other staff. Very humble and professional I guarantee patients are in good hands with her. Once again thank you for all you do.

My husband is there. When CNA Bianca and Nurse Tamiko is working my husband gets the best care. Ms Liz gets things done. But when Bianca and Tomika not there my husband seem to come with more problems. For some reason he doesnt get the same care. They need to get more like them or keep them 24/7. IJS Oh and getting the phone answered back there is next to impossible. Or if they answer I cant get the truth about how he's doing.

You call this place to talk to your loved ones and no one ever answers so you can never talk to them. And the “inmates”/ patients are not able to call out with the phones they give them so no loved ones are ever able to talk to each other. I have not been able to speak with my mother for 5 weeks now because no one will ever answer the phones! They are not open 24 hours a day!
