Horrible facility, trying to move my dad out ASAP. The only good thing about this place is the residents, who deserve so much more than they're getting. When I drop in to visit there's never anyone at the desk doing COVID screening like they're supposed to, the food is always served cold (and the microwave got taken away so no way to reheat the cold food. The staff is hard to track down when emergencies happen and doesn't seem to be trained in memory care at all. My dad has to do his own laundry because when the staff does it his clothes get lost, & the staff doesn't seem to care about the residents (at least not in memory care). There's no stimulation for residents or activities like they advertise. My dad has become incredibly depressed in only a few months of living here. New management has not helped this place one bit- save your loved ones, don't let them live here.

I worked at this place for a couple of days because Deena threw me to another side that I didn’t want to which was people with dementia , it was too much for me mentally and emotionally, I didn’t even know I was going to work on the other side so I quit because when I told her about it she said “sorry” and looked away at her computer screen and continued doing what she was doing. With that being said it has been 5 months now and I still haven’t received any check , but I received my W2 saying that I did get paid. Funny when I go and tell her she tells me they don’t own that company anymore and she doesn’t know anything, very unprofessional hiring manager, I’ve never experienced this in my life and I will have to speak to the main bosses or they will be sued, it’s very ILLEGAL to not pay a citizen the hours they worked for.

Watch where you sit, several times visiting I was sitting on couches with crusty messes all around. This facility lost my mother's clothing, had very poor administration communication and neglecting nurses. I would visit my mom only to find out her teeth hadn't been brushed with food in her teeth and her finger nails so long she was making scabs in her ears from trying to itch! Just don't even bother until they get their act together.

We have about 64 residents in the assisted living area, don't know how many residents in the Dementia section. We have on some days 4 caregivers because there is 4 sections, but sometimes we only have 2 caregivers it really depends how many caregivers show up in a day. We also have 2 med passers for the whole building. I don't know how many caregivers there is in the dementia section though. Just come over and check it out the staff will be able to give you better answers.
There are a variety of hotels in Forest Grove to rest including - McMenamins Grand Lodge, Best Western University Inn & Suites, Amercas Best Value Inn, and the Forest Grove Inn.
They offer assistant living and memory care options with different care options.
