Yes they are doing appointments only right now. You will need your ss card birth certificate. May not need both. Deff gona need proof of address, like a phone bill from the last 30 days. Also bring any photo I'd you might have from Florida.
You will need an appointment. The DMV website will tell you what documents. I believe Social Security card and two proofs of residency but we went for the real ID so you might get by with one.
