If the individual is unable to fill out the form themselves they are probably incapable of driving right? So if you're their caregiver I'm sure they have a service that provides for transportation and if not and you're providing the transportation then that would be between you and the company you work for.

The providers are allowed to charge for their services, of course. A separate fee for completing a form is not illegal. You should consider renewing in person. The form and exams are not required.
